I, as the parent or legal guardian of the above named child do hereby give permission for the camp, staff, or other assignee to take photographs or videos of my child while participating in camp activities. I understand that these photographs or videos may be used for promotional purposes, including but not limited to: camp brochures, websites, social media, and other forms of media. I understand that by submitting this form, I consent to the capture and use of my child's likeness in this way.

Camper is participating Crossfire's Summer Camp, a ministry of The Boulder Spirit Foundation. The Summer Camp is a gaming event for youth.
General Release of Liability
The Parent/Guardian and Camper acknowledge and agree that the Boulder Spirit Foundation, its employees, agents, and volunteers, shall not be liable for any injury, loss, or damage to the Camper or to any property of the Camper, whether such injury, loss, or damage is caused by the negligence of the Boulder Spirit Foundation, its employees, agents, or volunteers, or otherwise.
Transportation of Campers
The Parent/Guardian and Camper acknowledge and agree that transportation of the Camper to and from the Summer Camp, as well as transportation during the Summer Camp, may be provided by the Boulder Spirit Foundation or by independent contractors hired by the Boulder Spirit Foundation. The Parent/Guardian and Camper acknowledge and agree that the Boulder Spirit Foundation shall not be liable for any injury, loss, or damage to the Camper or to any property of the Camper that occurs during transportation, whether such injury, loss, or damage caused by the negligence of the Boulder Spirit Foundation, its employees, agents, or volunteers, or otherwise.
Indemnification
The Parent/Guardian and Camper agree to indemnify and hold harmless the Boulder Spirit Foundation, its employees, agents, and volunteers, from and against any and all claims, demands, liabilities, costs, and expenses, including reasonable attorney’s fees, arising out of the Camper’s participation in the Summer Camp.
Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of Colorado and/or Montana.
Entire Agreement
This Agreement constitutes the entire agreement between the parties, and supersedes all prior negotiations, understandings, and agreements between the parties, whether written or oral, relating to the subject matter of this Agreement.
